The type of exercise you do may affect dementia risk

Regular physical activity is widely considered beneficial for health, but new research suggests that where and why people are active may matter when it comes to dementia risk.

A large analysis involving more than 4.2 million people found that physical activity done during leisure time was associated with a lower risk of dementia, while physically demanding work was associated with a higher risk.

The study was conducted by researchers at the University of Southern California and examined evidence from 74 studies across 30 countries. The findings suggest that not all forms of physical activity have the same relationship with long-term brain health.

Dementia affects memory, thinking and the ability to perform everyday activities. Alzheimer’s disease is the most common cause, although several other conditions can also lead to dementia.

There is no guaranteed way to prevent dementia, but researchers have identified several factors associated with risk, including smoking, high blood pressure, diabetes, social isolation and physical inactivity.

Exercise may protect the brain in several ways. Regular activity can improve cardiovascular health and blood flow, while also helping manage weight, blood pressure and blood sugar.

However, the new research raises an important question: Is strenuous physical activity at work as beneficial as exercise people choose to do in their free time?

Leisure Exercise Linked to Lower Risk
To investigate the question, researchers analyzed 74 cohort and case-control studies involving adults between 45 and 93 years old. Participants were typically followed for about a decade.

Rather than treating all physical activity as the same, the researchers divided it into four categories: leisure-time exercise, occupational activity, household activity and active commuting.

The clearest association involved recreational exercise.

People with higher levels of leisure-time physical activity had a 24% lower risk of dementia compared with those who were least active during their free time.

The apparent benefit increased with greater activity before eventually leveling off, suggesting that more exercise was not associated with unlimited additional protection.

Physically Demanding Jobs Showed a Different Pattern
The findings for occupational activity were markedly different.

People who reported higher levels of physical activity at work had a 20% higher risk of dementia than those with lower levels of occupational activity.

The researchers stressed that this does not prove that physical movement at work causes dementia.

A physically demanding job is very different from voluntary exercise. Someone who goes cycling, swimming or walking for pleasure can usually control the pace, duration and intensity of the activity and take breaks when needed.

A worker, by contrast, may spend hours lifting, carrying, standing or performing repetitive tasks while having limited control over the pace or opportunity to rest.

Physically demanding occupations may also expose workers to other factors that could affect long-term health, including stress, noise, air pollution and limited control over working conditions.

Socioeconomic differences could also play a role. Education, income, access to health care and other lifestyle factors may differ between people in physically demanding and less physically demanding occupations.

The "Physical Activity Paradox"
The findings are consistent with what scientists have called the physical activity paradox.

Previous research has found that exercise during leisure time is often associated with better cardiovascular health, while high levels of physical activity at work do not necessarily provide the same benefits.

The USC researchers say their study is the first systematic investigation into whether this pattern also applies to dementia, and their findings suggest that it may.

One possible explanation is exhaustion. People who perform strenuous work for hours may have less energy or time for recreational exercise, social interaction and other activities that can contribute to overall health.

Evidence on Other Activities Was Limited
The researchers also examined household activity and active commuting, but the evidence was less conclusive.

Household physical activity appeared to be associated with potential benefits, but only one study provided enough information for a detailed analysis.

Active commuting was associated with a modest increase in dementia risk, but this finding was based on only two studies. The researchers said there was not enough evidence to draw firm conclusions about either category.

The Study Has Important Limitations
Despite its enormous sample size, the research cannot prove that a particular type of physical activity causes or prevents dementia.

Most of the studies relied on participants reporting their own activity levels, which can introduce errors because people may not accurately remember or estimate how much activity they perform.

Most of the research also came from high-income countries, limiting how confidently the findings can be applied to populations in lower-income regions.

And because people are not randomly assigned to different occupations or lifestyles for decades, other differences between groups could explain some of the results.

The researchers therefore do not suggest that people should avoid physically demanding work. Instead, they argue that physical activity at work should not automatically be considered equivalent to voluntary exercise.

What the Findings Mean
The research suggests that people with physically demanding jobs may still benefit from opportunities to engage in enjoyable recreational activity, get adequate rest and improve their overall working conditions.

For communities and policymakers, the findings could also have broader implications. Dementia prevention may require more than simply encouraging people to exercise.

Access to parks and safe walking and cycling routes, healthier workplaces and sufficient leisure time could all help people incorporate enjoyable physical activity into their lives.

The study, led by Professor David Raichlen and first author Natan Feter at USC, was published in The Lancet Public Health. Its large sample provides substantial evidence of an association between different types of physical activity and dementia risk, although it does not establish cause and effect.

The overall message is straightforward: being physically active remains important, but the benefits of movement may depend partly on the circumstances in which that activity takes place.
